Fish Oil Complex for Endometriosis: Research Findings and Implications (2026)

Endometriosis, a chronic inflammatory condition affecting around one in ten women of reproductive age, has long been a challenging diagnosis and treatment. The recent study published in Nutrients, titled "Effect of Fish-Derived Lipids on Inflammation Status and Health-Related Quality of Life in Women with Endometriosis," offers a glimmer of hope by exploring the potential of nutritional intervention. This research, conducted by scientists at Warsaw University of Life Sciences, Poland, delves into the impact of a fish oil complex on endometriosis symptoms and the underlying mechanisms involved.

The study, supported by Marinex International, involved 56 women with endometriosis who were randomly assigned to either a control group following a standardized healthy diet or an intervention group receiving the same diet supplemented with the fish-derived lipid formulation, Biomarine Medical Immuno & Neuro Lipids. The 12-week trial assessed various inflammatory markers, gut inflammation, pain, and quality of life (QOL).

One of the key findings was a modest improvement in pain and QOL in the supplemented group. However, the effects appeared context-dependent, suggesting an immune re-balancing rather than generalized immuno-suppression. The researchers noted that while there were consistent reductions in circulating pro-inflammatory cytokines, these changes were not statistically significant, and improvements in pain and QOL were heterogeneous across participants.

This study highlights the complexity of endometriosis and the need for larger, adequately powered trials with more specific endpoints. The authors emphasize that the results should be interpreted with caution and confirmed in larger studies with longer intervention periods, considering the potential mechanisms of biological response. This cautious approach is crucial to understanding the true impact of nutritional interventions on endometriosis.

The fish oil complex used in the study contains a complex mixture of approximately 500 bioactive lipids, including n-3, n-9, and n-11 fatty acids, alkylglycerols, squalene, and vitamins A and D3. EPA and DHA, widely recognized for their immunomodulatory actions, were found to increase in the intervention group. Additionally, squalene and alkylglycerols, which have been linked to anti-inflammatory effects in experimental contexts, also contributed to the selective immunological response observed.

The study's methodology, including the use of biochemical assessments and quality of life questionnaires, provides valuable insights into the potential benefits of nutritional intervention for endometriosis. However, the authors stress the importance of further research to fully understand the mechanisms and identify the most responsive individuals.

In the meantime, the UK's National Health Service (NHS) has approved two new non-invasive tests for endometriosis, a saliva genetic test and a gut sensor-pad test, aiming to reduce diagnosis delays. This development reflects a broader shift in women's healthcare, emphasizing the need for faster answers and earlier intervention. As research continues to explore nutritional interventions and diagnostic advancements, the future holds promise for improved management and quality of life for women living with endometriosis.
